Player Registration

Congratulations to all teams participating in the XIIth edition of Flag Day!

To ensure a correct and complete registration, please follow these instructions carefully: 

1. Add the team logo (Each team must add its official logo.)
   
   
2. Information about the club Fill in the name of the participating club Indicate the name of the person responsible. Specify the address and telephone number of the club. Select the registration category. 
   
   
3. Information about the players Fill in each blank corresponding to each player's first and last name. Indicate the player's date of birth (dd/mm/yyyy). Where they are born? The code will be defined by the committee. 
   
   
4. Pictures of the players Each player must provide a recent ID photo in JPEG or PNG format. Make sure that each picture is properly placed in the corresponding section. 
   
   
5. When you have completed the form, double check all sections before pressing "Click to submit"
